Services Overview
Holmesglen Hospital is proud to offer both inpatient and outpatient rehabilitation services.
Inpatient Rehabilitation
You may be referred to our rehabilitation ward during your inpatient stay at Holmesglen hospital, or from another hospital. Our rehabilitation team may be contacted to guide you through the process.
On admission you will be assessed by a multidisciplinary team who will establish an individualised plan for you. Depending on your condition, team members included in your care may include:
- Rehabilitation specialist
- Physiotherapist
- Occupational therapist
- Dietician
- Speech pathologist
- Discharge planner
- Diabetes educator
You will have access to our state of the art gym and kitchen space, and the program will help enable you to achieve your highest level of function, whether it be supporting you to live at home, return to work or achieving any other goals you may have. Your progress will be monitored through weekly case conferences.
Outpatient Rehabilitation
Our Day Rehabilitation Services is specifically designed to meet your individual needs. The programs are individually tailored and delivered in a group setting Mondays to Thursdays, in our state of the art gym.
We currently offer the following programs:
- Cardiac rehabilitation
- Pulmonary rehabilitation
- Orthopaedic rehabilitation
- Reconditioning rehabilitation
- Oncology Rehabilitation (coming soon)
Most groups are fully covered by your health insurance, and you can get a referral from your GP or specialist. For more information, please contact 03 9567 9090.
Suitable for patients who have recently undergone cardiac event or cardiac surgery, our phase II cardiac program aims to empower patients through lifestyle interventions, embedding exercise as a daily activity. In addition, education is conducted via the multidisciplinary team, providing patients with the skills and knowledge to live and thrive with their condition. Patients also develop peer support with a group with similar experiences, which has been shown to improve program adherence and effect.
Our pulmonary group is suitable for patients with long term breathing related diseases such as COPD, bronchiectasis and emphysema.
This program aims to improve an individual’s exercise tolerance, fitness and self-management through an exercise and education program with a focus on self-management. The program runs for 6 weeks and includes both strength and resistance training along with aerobic exercises using equipment such as treadmills and exercise bikes. The program covers education topics including medication, self-management, lung diseases, energy conservation, health eating, breathing techniques and airway clearance. The program has been shown to reduced hospital admissions, reduce frequency of exacerbations, improve exercise tolerance and quality of life. The program is overseen by a rehabilitation physician with input from exercise physiologists, physiotherapists, occupational therapists, dieticians and pharmacists.
Our orthopaedic rehabilitation is tailored for patients who have recently undergone orthopaedic surgery, including joint replacements, fractures, and spinal surgery. It focuses on building your endurance and aids return to your maximum function.
Our reconditioning rehabilitation is designed for patients who have recently experienced a serious acute illness, at risk of falling, and need to improve their strength, endurance and balance. Your recent episode of care does not have been at Holmesglen for you to participate in this program.
